  On 09/21/21, C1 misstepped/slipped off of a Director Tomashefsky stated that short-term, the Sunflower room will use the inside child-size sink and the Lavender Room will remove the step stool until it is needed for use and then a staff will grab it and supervise the child while they are using ethe sink. Director is proposing to purchase two child-size sinks to be utilized outdoors and will keep LPA informed of the one-step step stool, causing the stool to flip over, and C1 fell in between the legs of the stool and onto hard cement, sustaining a broken elbow. *This poses an immediate risk to the health and safety of the children in care.* progress of the proposal.
