Per Child Development Supervisor additional traning in the form of coaching, modeling on how to redirect children will be provided to staff#1. In addition staff #1 will view video on child's personal rights from CDSS.CA.GOV A written plan of the above will be provided to LPA by POC due date. Based on interviews conducted, the facility did not comply with the section cited above as evidenced by disclosures made during department interviews, which poses a potential risk to the health, safety, and personal rights of children in care. Director stated the facility is not using due to summer closure and Room H is not operational. The facility has put an "out of service" sign on and covered faucets J, E & F with plastic bag around the outlets making them inaccessible to children. Per Director faucets have been flushed and aerators Based on record review, the licensee did not comply with the directive above, as three outlets out of fourteen outlets tested with an Action Level Exceedance (ALE). This poses a potential Health and Safety risk to children in care. cleaned and has been retested. Director will continue to provide the status and updates regarding the formal report for the outlets that had been retested. Director will email LPA report within 30 days by 07/21/23.
