  Licensee has added a plastic safety door knob to the door next to the kitchen and front door and will install a safety gate to the right side of the home by June 30 2026 as a added measure to prevent children from exiting the facility. Due to the lack of supervision resulted in a child wandering outside of the facility and was found by a bystander who reported incident to the local law enforcement. This incident poses/posed an immediate risk to the health, safety or personal rights of the children in care.
