The Assistant Director is going to conduct a zoning and supervision training, staff will watch the Supervising Children in Child Care Centers on CCLD website and submit the staff summary of the video, and what they staff feel they can do to ensure the children are supervised at all times. Based on documentation and video footage, the licensee did not comply with the section cited above, as a child left through the playground door, and into the building unsupervised for approximately 1 to 2 minutes before the authorized representative noticed the child inside of the facility unsupervised, which poses an immediate health, safety, and personal rights risk to persons in care. staff and the Director The Director stated they repaired the play yard gate latch and conducted an in-service to staff on supervision due to the incident with child #1. The Director will provide proof of an in-service to staff on care and supervision and will submit proof of in-service by 05/11/2022. The Director will also submit a written plan on how the facility On 04/05/2022 at 4:27 PM, child #1 was left unsupervised behind the enclosed play yard gate for an undetermined amount of time until staff noticed the gate door was left opened and the latch broken. The staff went to retrieve child #1 behind the gate.
