Facility has already made correction to top gate. Director has reviewed drop off procedures with staff members and provided proof of it. On 2/27/25 Child (C1) wandered away from the drop off area. which poses a potential health, safety and personal rights risk to children in care. child managed to open the chain linked fence, pass through a gated area, descend a flight of stairs, to reach and push open the security fence. Staff member (S1) intercepted the child at the gate door below, preventing any harm from occurring. This poses a potential health, safety and personal rights risk to children in care. Director stated she will provide a training summary/agenda on supervision 8/30/24 to the Department by and conduct the training and provide sign-in sheet proof of all staff participation by 9/22/24. Based on the information obtained during interviews and review of pertinent documentation, staff did not provide sufficient care and supervision to prevent unkind interactions between children, resulting in C1 sustaining a minor injury by C2. This poses an immediate health, safety, or personal rights risk to children in care.

Director stated she and staff S1 will review the training on Children’s Personal Rights on CCLD training website https://ccld.childcarevideos.org/family-child-care-providers/ and provide a summary of their understanding to the Department by 7/1/24. Based on investigation interviews, staff S1 used inappropriate discipline practices by withholding food from children. Although food was never withheld from a student for an entire meal, a student may need to wait until all the other children were finished or have it delay as a form of disciple which poses/posed an immediate health, safety or personal rights risk to children in care.
