evidenced by Director stated, dance teacher is not contracted by the facility anymore. Staff members will conduct training regarding children`s personal rights , watch the video regarding children`s personal rights, active supervision.Meeting agenda will be sent to LPA by POC date. During the day care hours ,C1 was pinched by Adult1 contracted by the facility in the childcare center, which poses/posed a immediate health, safety or personal rights risk to persons in care. Director stated, she will have assigned staff member to provide active supervision all the time and classes will be conducted in small groups.

evidenced by Director stated repairs conducted on weekends or after hours when children are not present. Per Director, ceiling was closed and tiles fixed. Pictures will be emailed to LPA when everything is complete. Director stated, if any future issues with the classrooms , arrangments will be made to keep children safe and Department will be notified. During the interviews staff and parents confirmed water leaks were observed in the classroom and celling was open in one of the classrooms. LPA observed loose tiles and open celling in the classroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
