Licensee agreed to have S2 and S3 complete a Live Scan and will provide LPAs with completed copies of the Live Scan form by close of business on 01/24/2025. Based on observations, interviews conducted, and records reviewed, it was revealed S2 and S3 have been residing at the facility since 12/28/2024 without current criminal record clearances which poses an immediate safety risk to children in care. Licensee stated S2 and S3 will not return to the facility until criminal record clearances are obtained. A civil penalty of $200 is being assessed and provided.
