LPA reminded Director of criminal record clearance requirements. Director agreed to have S1 complete a new Live Scan and will provide LPA with a completed copy of the Live Scan form by close of business on 08/14/2025. Based on observation, interview conducted, and records reviewed, it was disclosed S1 has been working at the facility since 01/30/2024 without obtaining a criminal record clearance which poses an immediate safety risk to children in care. A civil penalty of $500 is being assessed and provided. Director understands S1 cannot return to the facility until a criminal record clearance is obtained.
