During records review LPA was not able to review 3 infants in care 15 minute sleep log. Licensee stated she does not document 15 minute safe sleep. which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 02/23/2024

Deficient Practice Statement Based on observation the licensee did not comply with the section cited above. At arrival to facility LPA observed 2 infants strapped in and sleeping in car seats. The 2 infants were covered with blankets and 1 infant had a pacifier with a lanyard attached while sleeping.which poses an immediate health, safety or personal rights risk to persons in care.
