Per HR Manger, the soccer clinics are conducted in a licensed Room 110. The HR Manager will provide a written statement by the POC due date. Based on 11 staff interviews, it was determined that on August 8, 2025, there was a soccer clinic for the children in a building that is not licensed.

Per director, they are currently using name to face on their app or parent app or they have a printed copy they have to look at the child's name and face. We also have multiple staff meeting concerns on how they are handling supervision. Director will submit a written statement to LPA of new plan by the POC due date. Based on staff interviews, C1, C2, and C3 were alone unsupervised on 2/24/25, and 2/28/25. This is an immediate risk to the safety of the children in care.
