Director will submit an email with information and list of staff who attended the training by POC due date December 14, 2022. Based on case report received from Irvine Police Department, Photos obtained showing injuries to C1, interviews with staff who witnessed Staff 1 (S1) pinch C1 causing injury to C1 face.
