Director stated that the facility has decided to no longer prop open doors for ventilation unless all children are moved to a different classroom. Director stated they will provide staff training documents with signatures and email them to LPA by due date. Based on interviews and record review, the licensee did not comply with the section cited above in that, Child 1 (C1) was in the play yard alone without teacher supervision, which poses an immediate health, safety or personal rights risk to persons in care. Director provided staff meeting with supervision training and a list of the signatures of the staff that attended the meeting and proof was sent to LPA on 10/05/23. Based on LPA interviews and record review, S3 failed to provide supervision for the three children. The three children were able to exit the facility to the parking lot. This poses an immediate Health and Safety risk to the children in care. A civil penalty of $500 was assessed today.

by Director stated staff #1 (S1) is no longer working at Everbrook Academy. Director stated they conducted a staff training on Engagement and Behavior as well as Hands off. Based on LPA observation of S1 picking C1 up by the wrist/arm, dragging C1, and handling other children in care innapropriately by pulling their legs/ankles to shift them on their cots, this poses an immediate Health and Safety risk to children in care.
