  A training will be conducted with the staff regarding supervision and observation. Statement and training signed by staff will be provided by the due date of 11/21/2025, 5pm. Verification will be sent to LPA Castro at the email address: alma.castro@dss.ca.gov Based on interviews and record review, the facility did not comply with the section cited above. None of the staff observed or witnessed the incident when C1 sustained an unknown head injury while in care which poses an immediate health, safety or personal rights risk to persons in care.
