Site Supervisor stated the Substitute Teacher will now be in the entrance of the classroom by the door to ensure children do not leave. Site Supervision will also submit a written plan by POC Due Date stating the changes made and a statement stating the Facility understands the importance of Visual Care and Supervision. Based on LPA interviews, Site Supervisor failed to provide care and visual supervision for C1. There was absence of supervision for about 50 mintues, Facility was unaware of his whereabouts. C1 was found to be home after he attended care, Facility was unaware C1 left home. This poses an immediate Health and Safety risk to the children in care. SUPERVISOR'S NAME: Rina Lopez
