Interviews with S1, S2, S4, S5 confirmed that S3 told the child that they cannot use the bathroom. Staff took toys from a child and threw them hitting staff and ceiling. This poses an possible health, safety, and personal rights risk to children in care. Based on LPA interviews, it was confirmed that children's personal rights were violated by a teacher hitting a child. Interviews confirmed punishment of child. This poses an possible health, safety, and personal rights risk to children in care. Principal has corrected this deficiency by adjusting staff schedules, hiring a teacher, more frequent ratio counts are completed throughout the day and each count is logged. Based on interview and record review, Principal did not ensure Title Five ratio was met. Classroom had 30 children supervised by one qualified teacher and four assistants, putting the classroom out of ratio by six children. This poses a potential health and safety risk to children in care.
