1 Staff stated that S14 no longer works at facility and gave document supporting S14 no longer is working at facility. Based on records review and staff interviews, LPA observed video footage and Unusual Incident Report (UIR) of S14 forcibly laying C2 on cot. 2 (two) staff interviews stated S14 forcibly laid C2 on their cot. This was not met as evidenced by New diaper changing policy has been put in place on 11/01/2022 that require staff to conduct diaper checks after every dailiy activity transition. Director agrees to conduct a retraining regarding diaper changes and regulation being addressed. LPA interviews with Reporting Party, Asst. Director and Staff #1, it was discovered that subject child did not have a diaper check or change for a long period of time. Staff stated the child had just returned from the playground and was about to have their diaper changed when RP picked them up, 2.5 hours after their last diaper change, causing a rash on the child. Children need to be changed as needed. This poses an immediate risk to the personal rights of children in care. Director will provide LPA with a training agenda and role sheet of all infant staff scheduled to attend. Attendance sheet will include staff signatures. Director will email copy of agenda and role sheet to LPA at romelia.castanon@dss.ca.gov by specified date 12/16/2022.

Director stated they went over supervision policies with all staff on 6/28/22 and each staff signed the supervision policy. They have also implemented supervision zones in the outdoor playground. Director stated they will provide a written statement with supervision POC by 8/10/22. Based on interviews from Staff #2 and Staff #3, they did not observe when C1 was bit on the right arm and left leg by another child. They believe it happened outside in the playground and during the fire drill, but they disclosed they did not observe when the incident occurred. This is an immediate risk to the health and safety of children in care.
