The Center Owner will submit the required LIC 308 form and all required documentation to appoint a new Director or Assistant Director by the POC date requested of 10/6/23. Based on interview and record review, LPA did not receive the required documentation within 10 days of the newly appointed Assistant Director at the facility which poses a potential health, safety, or personal rights risks to the persons in care. Per Site Supervisor stated they will be sure to call in incidents immediately and file necessary paperwork within 24 hours. Site Supervisor stated they will provide a written declaration to Licensing by chosen POC date of 8/18/2023. Based on interviews and record reviews, there were injuries and inappropriate behavior that was not reported to Licensing which posed a potential health, safety, or personal rights risk to the children in care. The Licensee will bring their center back into compliance by only allowing the number of children specified on their license in attendance and will submit a written statement that they will remain in compliance with capacity regulations no later than 6/29/2023. Based on observation, interview, and record review, the Licensee did not comply with the section cited above for the capacity limitations by having 28 preschoolers in the outdoor play area which poses an immediate health, safety, or personal rights risk to persons in care. The Licensee will bring their center back into compliance by observing the teacher-child ratio specified on the license and will submit a written statement that they will remain in compliance with teacher-child ratio regulations no later than 6/29/2023. Based on observation, interview, and record review, the Licensee did not comply with the section cited above by having 28 preschoolers in the outdoor play area being supervised by 2 staff exceeding the teacher-child ratio which poses an immediate health, safety, or personal rights risk to persons in care.
