101229(a)(1) · (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This is not met as evidenced by SS stated that a number pad or key card pad will be installed on the outside of the door to prevent unsupervised entry into the classroom. On 2/12/26, C1 was unsupervised in the classroom for approximately 1 minute. Staff did not observe C1 enter the classroom by themself which poses a potential risk to the health and safety or personal rights of children in care. SS has reviewed supervision rules with the staff and do weekly review as well. The teacher assigned to the sensory table near the door will be monitoring access. SS will provide photo verification to jennifer.patel@dss.ca.gov of key/card pard installation. staff interviewed indicated that milk was not served on one day in mid January 2025 dues to milk being spoiled and the program ran out of fresh fruit Facility has regular food orders which can be supplemented with purchases from Costco and Safeway online as well as Instacart if any food item is needed immediately. New staff have been made aware of the availablity to clear the deficiency at the time of visit. This poses a potential health and safety risk to the children in care.

101212(d) · Reporting Requirements- upon occurrence of an unusual incident a report shall be made to the department by telephone within the next business day during business hour and a written report shall be submitted within 7 days following the occurrence. This requirement has not been met as evidenced Unusual Incident Report was submitted to the department. The delay in reporting was that the former site supervisor who was terminated did not notify the YWCA Program Office of the incident until after the regulatory time of reporting had been exceeded. by: the incident occurrence date was 1/30/25 and the department was notified with a unusual incident report on 02/18/2025.
