Facility will conduct a review of supervision training and submit record of training to LPA by POC due date. Based on interviews, while sitting at a table inside classroom, S1 had turned away from the children while moving a tub of supplies. During this time, C1 and C2 were unsupervised, and an incident occurred in which C2 cut C1’s hand with scissors, resulting in an injury. S1 left C1and C2 without visual supervision by a facility staff, which posed an immediate Health and Safety risk to children in care.
