  101223(a) · 101223(a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons.

  Facility had a Behavior Action Plan in place prior to and at time of incident. Facility has held follow up conferences with staff and other involved parties to discuss protocols to ensure the health and safety of all children, as well as support planning for Child #1. Based on interview and record review, it was determined that Child #2 was injured by Child #1. This poses a potential risk to the health, safety, and personal rights of children in care. LPA obtained copies of these plans and Site Supervisor stated facility will continue to reassess support plans as often as necessary, and continue to shadow Child #1. POC cleared today, 05/03/2023.
