102416.2(a) · 102416.2 Reporting Requirements (a)The licensee shall report the following information the Department by telephone or fax within the Department's next business day and during normal working hours (8am to 5pm).

LPA provided licensee with LIC 9211, Request for Inactive Child Care License, during today’s case management inspection. Licensee completed and signed the form during the visit. LPA also informed licensee that, due to the facility being licensed as a Large Family Child Care Home, fire clearance must be obtained upon completion of construction and prior to the facility returning to active status. Based on observations and interviews conducted, the licensee failed to notify the Department of a water leak in the home and the construction repairs needed as a result of the leak in areas utilized for child care. Licensee stated the facility plans to remain closed for approximately two to three weeks while repairs are being completed.
