Director states she had set traps at the time of observing the rat feces and urine and as of this date is cleared of any rodents. All items that were observed to have signs of rats were disposed of. Directors states she will have a cleaning log for the staff to use to document when cleaning is conducted and as evidenced by interviews conducted and LPA observations that the facility had a issues with rats in the facility.
