Per licensee, staff#4 was given written disciplinary action, staff#4 watched CCL Personal Rights training on line. Also a meeting will be held with staff on 07/03/25 and a copy of staff sign in sheet and topics that will be discussed will be sent to LPA Eivazian. Per four staff interviews, on 06/23/25 child#1 had thrown himself on the floor, where staff#4 picked child#1 up from underarms at which time the bruise would have occurred as child#1 was resisting. Alleged Incident took place on 06/23/25. Licensee emailed the written report to case caring LPA Eivazian on 06/30/25. LPA asked licensee to report the incident to officer of the day. Incident was reported to the Department via email on 07/01/25. Based on four staff interviews, on 06/23/25 child#1 was picked up by staff#1 from underarm which cause bruise on child#1 right arm and this incident was not reporter to Licensing Department within 24 hours. This poses a potential health, safety and personal right risk to the children in care. The facility did not report the incident within the required 24 hour time frame. Per Bhagya Fernando, owner, he will meet with staff to ensure to treat each child in a safe manner and if it will be needed to elevate the case to the director and discuss it with parents. A written plan will be submitted to LPA. Based on an interview that was conducted with staff#1, staff#1 lifted child#1 from floor from armpits (date unknown), to prevent child#1 to go under the table and placed child#1 on the chair and stated sit. This poses a potential health, safety and personal right to the children in care.
