Per director, Roxanne Mullich staff#3 was fired immediately after this incident on 08/01/25, an email was sent out to all involved parents on 08/01/25. A written plan will be submitted to LPA by 08/06/25. Based on Unusual Incident report, received on 08/04/25, and two staff interviews, on 08/01/25 at 4:00 p.m. staff#3 had fallen asleep with children in LadyBug classroom and was observed by staff#2. This poses an immediate health, safety and personal right risk to the children in care. Per director, pest control company sprays the facility monthly. Effective 05/22/23, daily cleaning crew clean the entire facility and gym. Per director, one staff will do walk through of the gym daily and every time before children to go to the gym. During this inspection LPA Eivazian observed one dead cockroach in the gym. This poses an immediate health, safety and personal right to the children in care. $250 Civil Penalty was assessed for Repeated Violation within 12 months. A written plan will be submitted to LPA by 07/14/23.

Effective today, daily one staff will check the gym, courtyard, and outdoor play areas to ensure it is clean and free of rodent, insects and cockroaches. During this inspection LPA Eivazian observed four dead cockroaches in the gym. This poses an immediate health, safety and personal right to the children in care. A written plan will be submitted to LPA by 05/23/23 via email.
