Director agreed to compose a letter on section 101229- Responsibility for Providing Care and Supervision and have all preschool staff review, sign and date the composed letter acknowledging the regulation. Based on interview, the Director did not comply with the section cited above by having C1 unsupervised in the toddlers playground during transition from playtime to lunch which posed a Immediate health, safety, or personal rights risk to persons in care. Director has also implemented training on supervision and how to avoid from happening again. Director informed staff to conduct name to face check by lining up the children, checking off names of children present during day care, and scanning the entire playground to identify children that have not been counted for. Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. LPA observed that in Room 4 there is cabinet below the sink that had cleaning supplies and bottle of bleach that was not lock, which posed a potential health, safety or personal rights risk to persons in care.
