Director conducted an all staff personal rights training on 1/13/2026 and an email sent along with a follow up email to LPA as a copy. In addition to the POC, Director stated there will be a continued HR investigation and will update LPA with the outcome on the investigation. Based on interviews, the licensee did not comply with the section cited above when Staff 4- S4 violated C1 personal rights by telling other children C1 had broken all his Christmas toys, which poses a potential health, safety, or personal rights risk to persons in care. Director agreed, starting September 19, 2024, teachers will be attending training for active supervision prodeedures, and this will be on-going monthly. Side director will be monitoring training on a hourly basis which includes out child supervision policy, prodeedues and responibilities of our active supervision protocal. Based on interview, the licensee did not comply with the section cited above by leaving C1 unsupervised and unattended outside of his classroom which poses an immediate health, safety or personal rights risk to persons in care.
