The director stated she already addressed the issue with S1 on 01/28/2026, and S1 had agreed that S1 will not touch, push or pull the children in care again in the future. The director will provide the personal rights training to all staff again and email LPA the confirmation letter on or before 3/31/2026. Based on the self-reported unusual incident report, observations, and interviews with 3 staff, it was determined that S1 was pulling C1’s left hand aggressively to make C1 stand up and walk which caused C1 to injure C1’s left wrist, which poses an immediate health, safety or personal rights risk to the persons in care.
