Based on observation, interviews, and medical records review, staff who were present at the playground did not provide adequate supervision: did not see/prevent C2 kicking C1, causing C1 to be injured. This poses a potential risk to the health, safety, and person rights of the children in care. The licensee did not meet the requirement above The school staff sought prefessional help from external resources agencies. It was ultimately decided that the child's needs were better served at a different educational institutions to which the child was referred. The child enrolled in the new school in March.
Based on interviews and records review, the licensee did not meet the regulation, which posed/poses an immediate risk to the safety and personal rights of clients. C1 hurt other children & touched them inappropriately while in care. Incidents' dates range from 10/2023 to 2/2024
