Facility Reprsentaive provided a written statement stating the facility provided training for all staff on 12/10/25 regarding face to name and the importance of supervision and safety. Based on the information gathered from the interviews conducted and statements received, it was determined that C1 was left alone on the playground unsupervised. This is an immediate risk to the health and safety of the children in care. Director will provide staff meeting with supervision training and a list of the signatures of the staff that attended the meeting and POC will email to LPA by the POC due date. Based on LPA interviews and record review, the facility staff failed to provide supervision to C1. C1 was able exit the facility, out the front entrance door to the curb near the drop off and pick up zone an approximate 151 ft. 101238(a) · 101238 Buildings and Grounds (a) The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
The school director indicated that on the day the incident, the facility immediately inspected the fence and used zip ties to secure the fence temporally; a technician was out the next morning to check on the fence, closed the gap and repaired the fence. The director indicated that all the safety measures have been taken Based on interviews and record review, on 08/17/2021, C1 went through unsecured fence; got out of the facility for approximately 5-10 mins; facility failed to keep the fence in good repair this lead to the incident. This poses an immediate risk to the health and safety of the children in care. including repairing, training, and reviewing the safety policy with all the staff in school to prevent any future incident. During the inspection on 10/06/2021 and 11/18/2021, LPA Nguyen observed the repaired fence. The deficiency is clear during today inspection.

The director will immediately conduct a saftey sweep training for outdoor play and saftey with all staff and provide a signed copy of the training. The facility director will also review supervison protocols with staff ensuring that supervison is in place for all children at all times. A copy of this will be provided to CCL by 8/26/2021. Based on interviews and statements provided, the facility staff failed to supervise the two year old child on the playground whereby the child was able to slip away out of the Teachers sight. This poses an immidicate risk to the health and safety of the children in care. 11/18/2021, LPA Nguyen accessed $500 civil penalty for absent of supervision. SUPERVISOR'S NAME: Thuy Ho
