The Director states Director will ensure all gates are locked and secured and will provide retraining for staff regarding supervison for the children in care at all times in the facility, and will submit proof of training and a detailed plan to ensure children will not be left unattended, to LPA by due date. Based on observations, interviews and record review, 4 of 4 interviewed staff admitted 4 of 6 unattended children, wandered away from the facility into a busy street. This poses an immediate risk to the health and safety of the children in care.
