Based on the interviews conducted, It was determined that C1 sustained electrical burns from an electrical outlet in caterpillar room 207 due to staff was not actively superving children's activity. Which poses an immediate risk to the health and safety of the children in care. The facility will submit a written plan to ensure children are not left unsupervised. Written plan to be submitted via email or text message by the due date of 11/27/23 Based on the information gathered from the interviews conducted, It was determined that C1 was left outside by the playground unsupervised for about 2 minutes.
