Director stated that they will have a staff meeting to review Children's Personal Rights, Alternative Strategies for Support, and Social Emotional Support. Director will provide CCL a copy of the agenda by the end of business day on 08/12/2025. Based on staff interviews, it was determined that Staff #3 held Child #1 by the arm and in an aggressive tone stated, “You are not listening.” Staff #3 then grabbed Child #1 by the chin, directing the child’s face toward Staff #3. This poses a potential risk to the health, safety and personal rights to children in care. Supervision shall include visual observation Director Barbara Daniels stated the center has since implemented face to name recognition policy to aid with transitioning and ensuring no children is left behind unsupervised. Furthermore, Director stated the facility will be conducting training pertaining to regulation 101229(a)(1).
Information received through record review and interviews indicate that a child was left unsupervised in a classroom for approximately 5 minutes while a fire drill was being conducted by the facility. Program Manager stated she will be having a training on active supervision before the end of the month 11/30/23 and Program Manager will submit to LPA proof of training.
