Based on video review, Site Supervisor, Vang will provided a copy of the training that will be conducted on 08/22/2025. The training is regarding active supervision and classroom transitions. The Site Supervisor stated that she will also include the training for all new staff hired going forward. LPA Herrera observed child #1 was left with an absence of supervision for approximately 4 to 5 minutes in the infant outside play area, which poses an immediate risk to the health, safety or personal rights of children in care. An immediate $500 civil penalty is being assessed. Furthermore Site Supervisor, Vang will provide a statement as how she will implent the training during new hire onborading and monthly staff meetings. The facility will ensure to provide the statement along with training documentation by 08/22/2025.
