Site Supervisor stated a training will be conducted on 10/13/2025 regarding active supervision with all staff and will provide sign-in sheet of attendees to Licenisng by 10/16/2025. Based upon staff interviews, Staff entered the classroom without a child and realized there was a child missing after entering another classroom. This violation poses a potential risk to the health, safety, and personal rights of children in care.
