Deficient Practice Statement Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed playstructure is missing two bolts from the right side hood of the slide and the left side has a large crack. There is a shed located on the play yard that is warped showing splinters and nails sticking out on the left side. As you walk around the shed there are visable nails sticking out from the back of the shed. There is missing fencing allowing access to the back of the shed. POC Due Date: 07/09/2025
