During the indoor inspection, Licensing Program Analyst (LPA) observed that the main blue carpet in Clasrooms 1 and 2 had dark marks and staining throughout. LPA observed some dead insects/flies along the bottom of the windows/window sills in Room 2. LPA also observed some ants on the cubbies near the front entrance in Room 1 and around the outside of the door in Room 2. Lastly, LPA observed a strong odor of urine coming from the bathroom in Room 1. POC Due Date: 12/15/2025
