The door to the kitchen area is now locked at all times. Facility also installed baby gates in front of the two doors leading to the outside area and latch locks to door handles.
Based on interviews, it was confirmed that there was an absence of supervision as a child exited the classroom and was located in a gated outdoor area. On 04/24/2026, Facility conducted a training regarding supervision in the classroom, playround, and bathroom. LPA was provided with a copy of the agenda, powerpoint of information covered, and a sign in sheet for the staff that attended. POC cleared on today's date and provided to Site Supervisor.
