Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above as LPA observed the play structure located on the north side of the playground had exposed metal on the handlebars and rusted metal on the steps. LPA also observed that the play structure in the middle had stepping areas that were cracked and lifting. Additionally, LPA observed that the grassy area in the middle, where the trees are located, had roots protruding above ground, which poses a potential health, safety or personal rights risk to persons in care.
