Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in the outdoor activity space. There are several fence post bases where the cement has broken off and there is exposed metal. On one metal post base, a cement block is detached. The storage shed has nails protruding from the trim of shed. The shed’s bottom panels on all four sides are jagged, some sides warped, chipped and splitting. Grass is growing in the wood chip area. The exterior wall, facing the shed, has a large piece of stucco missing. This poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 09/06/2023

(b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. (c) If testing indicates an Action Level Exceedance at any water outlet, the water from that outlet is deemed not safe to drink and an immediate response pursuant to section 101704 shall be required. Authorized representatives have been notified. The water to the drinking fountain was turned off and will not be used. A maintenance request was made to change out the drinking fountain outlet and replace the fixture.
