  Center Director Ybarra will conduct a formal training regarding supervision with staff on 05/27/2025 and will provide a sign-in sheet of attendees to Licensing on 05/28/2025. Center Director Ybarra stated sign-in protocols have been changed to ensure supervision is maintained at all times. Based on interviews with staff, it was revealed that Child #1 walked to the restroom located inside the classroom unsupervised. This lack of supervision poses a potential risk to the health, safety, and personal rights of children in care.
