Deficient Practice Statement During the inspection LPA Herrera observed that the two classrooms had soiled and stained carpets. LPA Herrera observed that the facility use the carpets for circle time. Furthermore, toilet #4 (biggest stall) did not have caps which exposed the metal bolts.
