Deficient Practice Statement During today's inspection, LPA observed two rooms that are separated by a door that the facility uses for daycare. The door remained open for the children to roam through both rooms. LPA observed five children, the Director and Staff #2 to be in one room at the same time for a short period of time, leaving the other three children left unattended in the other room. The children were not under supervision, including visual supervision, of a teacher at all times. Based on observation, the facility did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. POC Due Date: 01/24/2024
