101212(d)(1)(c) · REPORTING REQUIREMENTS - Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Dept. by telephone or fax within the Dept. next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Events reported shall include the following: Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Director will submit an Unusual incident report with details of the mold incident that occurred at the center on 8/13/2021. Director will develop a written plan to ensure all unusual incidents are reported

staff interviews. The investigation revealed that on 8/13/2021 mold was found in the infant classroom. Staff failed to notify CCL. Additionally, authorized representatives were not notified in accordance with Title 22 regulation 101212(f). This poses a potential risk to the health, safety or personal rights to children in care timely to CCL and authorized representative are also notified. Written plan is to be submitted to the Fresno CCL office by 4/29/2022.

101629.1(a)(b) · SIGN IN AND OUT - Notwithstanding Section 101226.1(b)(2)(A), and in addition to Section 101229.1, the following shall apply: The licensee shall require that each child be signed in and out by his/her authorized representative. As of 12/4/2021 parents and authorized representative are solely signing their children in/out from the center. Director agreed to develop a written plan addressing how she will ensure parents/authorized representative and not staff sign in/out day care children. This plan is to be submitted to the Fresno Community Care Licensing Department by 4/29/2022. During the COVID-19 pandemic Staff were signing children in/out from the center which is the responsibility of only the authorized representative. This poses a potential risk to the health, safety or personal rights to children in care.
