Based on LPA record review, the licensee did not comply with the section cited above. On 04/25/2025, Director Roxan submitted copies of transcripts Director Roxan Tutelian reviewed regulation CCR 101216.1(b) and stated she will immediately ensure that all staff utilized as teachers and teacher aides meet the regulatory requirements as of start of business 5/27/2025. Director stated she will create a classroom staffing plan that meets and work experience documentation of the 16 staff present on 04/11/2025 and it was determined that 3 staff met requirements as qualified teachers along with 3 as qualified aides. This poses an immediate risk to the health, safety or personal rights of children in care. all regulatory requirements for teacher/teacher aide requirements and ratios for all classrooms by 5/30/2025. LPA will conduct an unannounced inspection after 5/30/2025 to review classroom staffing plan and ensure that staff providing care and supervision are qualified per Title 22 regulations.

101170(e)(1) · Criminal Record Clearance. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. This was not met by evidence of staff record review annual inspection teacher Annelise Lyman was not finger printed. cleared. Teacher Lyman left immediately and went to go get finger printed. Licensee stated that teacher Lyman will not return to the center until she is cleared and associated to the facility. Licensee will send proof of finger print clearance and association. Therefore, a $500.00 civil penalty is assessed ($100.00 per violation per day for a maximum of five days by the Department). Unfinger printed teacher left the center during today's visit and another fingerprinted adult came to the class to assist. Licensee was given the Live Scan form during today's visit. Licensee to send a copy of the completed Live Scan form/receipt by 2/13/18 showing that assistant was fingerprinted. Licensee understands that her assistant cannot be present in the home until fingerprint cleared or have a clear exemption and is associated with her facility. This poses an immediate threat to the health and safety or personal rights of children. A civil penalty was assessed for $500 due to teacher has been working at the school for longer than 30 days.
