On 3/4/26 LPA observed S3 (substitute aide) Director shall submit by the POC date a written plan of action detailing how they will prevent this from repeating again. Facility was in ratio today. supervising a group of 11 preschoolers in room 2 without a fully qualified teacher present which poses an immediate risk to the health and safety of children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
