The Facility shall watch the "Child Care Reporting Requirements" at www.ccld.childcarevideos.org and submit a written statement explaining when and how an event should be reported to the Community Care Licensing Division (CCLD) by the POC due date of 02/27/2025.

Based on interview and record review, the licensee did not comply with the section cited above as there is no record of the incident reported to Community Care Licensing Division (CCLD), which posed a potential health, safety, or personal rights risk to persons in care. In the future, Licensee will report Unusual Incidents/Injuries to CCLD. Licensee will also file reports into personnel/children's files.
