Director is to provide a detailed report on changes implemented to prevent lack of supervision occuring in the future. Director has already conducted training on supervision. Agenda and staff signatures are to be provided no later than 6/26/2026. This requirement was not being met as evidence by: Self-reported incident of a child in care standing in the hallway for approximately 2 minutes during bathroom transition time. This poses a potential health and safety risk to children in care. LPA advised Director to develop a plan of correction and submit proof to the Oakland Office via email or mail on or before the POC due date. The Plan shall describe how the licensee will bring their facility into compliance. It must be measurable and verifiable. During the visit, Director Ware provided copies to LPA, of the facility's POC documentation, which included Transition Policy and Procedure, Supervision and Transition Audit, and a future dated Staff Meeting Agenda on 9/24/2025.

Director Ware went to the outdoor play area and found C1 inside a play structure, unsupervised. This poses an immediate and potential risk to the health, safety, and or personal rights of children in care LPA recommendations and requirements are the following, 1. The Technical Support Program (TSP). TSP is FREE, voluntary, and a non-enforcement service of Community Care Licensing to assist child care providers experiencing compliance challenges. 2. Today's reports will be endorsed to the local Resource and Referral for review. 3. Submit a copy, 9/24/2025 staff minutes of meeting to LPA. 4. Licensee to continue reaching out for resources to local Resource and Referral via Mychildcare.org.
