101229(a) · (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This was not me as evidenced by S1 stated she will make sure a child is offered a blanket, towel or something to cover up to stay warm. S1 stated they will always have 2 staff to support a child with meeting their needs. Based on staff interviews C1 was without clothes and was not properly cleaned for approximately 45 minutes. This posed an immediate health and safety risk to children in care. S1 will send LPA the plan to staff and assist children in the bathroom when necessary to Melinda.mohr@dss.ca.gov

This was not me as evidenced by S1 stated she will look into having the changing table moved to a different location not in front of the door and /or a smaller changing table purchased. S1 will email LPA a picture of the new location of the changing table at Melinda.mohr@dss.ca.gov Based on staff interview C1 was not provided comfortable accommodations as the bathroom was cold and C1 did not have privacy while being changed. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care.

Site Supervisor stated S2 no longer works at the facility, their last day at the facility was 10/31/2024. S1 knows the importance of this incident and that it has never been an issue and cannot happen in. S1 stated her first job is to keep the children safe.

based on an Unusual Incident Report (UIR) submitted by the facility on 04/16/24, It was reported by the site supervisor that during afternoon snack time on 04/12/24 at 03:15pm, child C1, who has a peanut allergy, ingested peanut butter that was unintentionally served by staff S1 and required medical attention. This posed a potential health and safety hazard to the child in care The site supervisor will submit a copy of the meeting agenda and staff sign in sheet to the LPA by 05/10/24.
