LPAs observed all 3 gallons of milk in the refrigerator were expired as of 08-06-24 & 07-14-24. During the inspection, LPAs observed licnesee changing out the groceries and confirmed that they had been replaced before the end of the day. LPAs also observed multiple “Honest” juice boxes with a “best if used by” date of 04-01-24. Additionally, there was a bag of “LandO’Frost” turkey breast meat slices with a label of “sell by” 08-02-24. LPAs also found mustard with a date of 07/26/2024.

102370(d)(1) · ***This is an amended report, the original report is dated 05/29/2024*** 102370(d)(1) All individuals subject to a criminal record review....shall prior to working, residing, or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption Licensee agrees that both uncleared adults will not return to the facility until they have a criminal record clearance. Licensee agrees to provide proof of a completed LIC9163 for each adult to LPA by 05/30/24 via email. A civil penalty of $100.00 per day, per adult, is assessed, totalling $1,000.00. as required by the Department.
