Deficient Practice Statement Based on LPA's observation, there is a wood play stucture outside in need of repairs. Currently the play structure is not being used and there is no ladder granting access to the structure. The licensee did not comply with the section cited above in that the structure has not been repaired or removed, which poses a potential health, safety or personal rights risk to persons in care.
